patch 9.0.1390: FOR_ALL_ macros are defined in an unexpected file

Problem:    FOR_ALL_ macros are defined in an unexpected file.
Solution:   Move FOR_ALL_ macros to macros.h.  Add FOR_ALL_HASHTAB_ITEMS.
